Andrew's Gamepad Plus Accuracy

DXHR has great standard gamepad support, but I prefer the High Accuracy configuration. Unfortunately DXHR has an awkward studder between gamepad and mouse/keyboard modes and is unusable as such. This is basically replicating the gamepad controls, but using keyboard mappings for better accuracy. (with a couple small changes because the gamepad has slightly differently functioning bindings) :)
